Medical History & Immunization Record

All incoming freshmen, including those participating in Freshmen Connection, must complete the University of Maryland Medical History and Immunization Record. Failure to submit the completed health form to the University Health Center before the first day of Fall 2008 classes will result in a registration block on your student record, preventing you from registering for the Spring 2009 semester.
